The Program Coordinator is responsible for leading the development, implementation, and daily operations of a supportive housing program designed for individuals with behavioral health and housing stability needs. This role involves direct oversight of staff, regulatory compliance, crisis response, and coordination with internal departments and external partners.

Program Coordinator Responsibilities:

· Assist with the supervision and coordination of direct care and crisis respite services.

· Conduct weekly individual and group supervisory meetings with staff, including completing weekly chart review.

· Provide training to assigned staff, as needed.

· Conduct screenings, assessments, and intakes of prospective applicants.

· Maintain program occupancy rates, assisting with the intake process, collection of supporting documentation and admission process.

· Monitor/review all documentation to ensure timely submission, qualitative content, etc.

· Provide crisis intervention (including on-call responsibilities).

· Compliance with NYSOMH, LIHTC, and other regulations, including data collection, and reporting requirements.

· Cooperation with Property Management regarding site maintenance.

· Acts as a liaison with other service providers, as well as with community agencies. Responsible for assisting staff and guests with crisis intervention and linkages to community resources

Program Coordinator Qualifications:

  • Master’s Degree and License in Social Work, Rehabilitation Counseling or other related field, minimum of three years experience working in a similar setting/ or working with people with mental illnesses.
  • The individual in this position must have good leadership and supervisory skills, good verbal and written communication skills, and the ability to make sound judgments regarding guest care.
