💼 What You’ll Do:

• Perform initial evaluations and develop customized Service Plans
• Oversee caregivers and ensure high-quality, compliant care delivery
• Supervise, train, and support your team while managing caseloads
• Coordinate client/family meetings and lead care conferences
• Take the lead on charting, assessments, and annual evaluations
• Ensure regulatory compliance and participate in quality reviews
• Occasionally provide skilled nursing services (wound care, catheter care, etc.)

📍 This role is perfect for you if you:

• Are a licensed RN with at least 2 years of home care experience
• Have 1+ year of supervisory or leadership experience
• Love working autonomously and with purpose
• Are organized, tech-savvy, and a natural communicator
• Have a growth mindset and a strong sense of compassion
• Want to work in a supportive, mission-driven culture

🚘 Must have a valid RN license, driver’s license, and reliable transportation. Experience with wound care, catheter care, and elderly care is a must!
