💼 What You’ll Do:
- Perform initial evaluations and develop customized Service Plans
- Oversee caregivers and ensure high-quality, compliant care delivery
- Supervise, train, and support your team while managing caseloads
- Coordinate client/family meetings and lead care conferences
- Take the lead on charting, assessments, and annual evaluations
- Ensure regulatory compliance and participate in quality reviews
- Occasionally provide skilled nursing services (wound care, catheter care, etc.)
📍 This role is perfect for you if you:
- Are a licensed RN with at least 2 years of home care experience
- Have 1+ year of supervisory or leadership experience
- Love working autonomously and with purpose
- Are organized, tech-savvy, and a natural communicator
- Have a growth mindset and a strong sense of compassion
- Want to work in a supportive, mission-driven culture
🚘 Must have a valid RN license, driver’s license, and reliable transportation. Experience with wound care, catheter care, and elderly care is a must!
