About the Opportunity

We are seeking a dedicated Primary Care Provider to join our dynamic, mission-driven team. In this role, you will play a key part in delivering high-quality, compassionate care to underserved populations, while practicing in an innovative, supportive environment.

What You Will Do

As a Primary Care Provider, you will:

• Provide a full scope of primary care services, with a strong emphasis on improving access and outcomes for underserved communities.
• Work within a forward-thinking clinical model featuring Advanced Access scheduling and robust clinical support services.
• Collaborate as part of a multi-disciplinary care team, delivering both individualized care and population-based health services.

Qualifications – Physicians (MD/DO)

• Medical degree (MD/DO) from an accredited medical or osteopathic school.
• Completion of an accredited family practice residency program.
• Board certification (or board eligibility), with certification required within 24 months of employment
• Active Connecticut medical license or eligibility for CT licensure.
• Bilingual English/Spanish skills strongly preferred.

Advanced Practice Providers (NPs)

• Nurse Practitioners are encouraged to apply and will be fully considered for this position.
