About the Role

A respected hospice organization is seeking an experienced Hospital-Based Community Liaison to drive admissions through acute-care hospital referral relationships in the Fort Worth area. This field-based role partners closely with hospital case managers and discharge planners to support timely, appropriate hospice transitions in a Medicare-driven environment.

The ideal candidate has established hospital relationships, understands hospice eligibility and compliance, and consistently produces admissions.

Key Responsibilities

Build and maintain strong hospital referral relationships (case managers, social workers, discharge planners)
Drive hospice admissions and census growth from hospital settings
Educate hospital teams on hospice services, eligibility, and transitions of care
Coordinate referrals with intake and clinical teams for timely admissions
Track referral activity and admissions performance
Maintain strict compliance with hospice referral regulations

Qualifications

Required

Hospice or home health liaison / business development experience
Existing hospital referral relationships
Proven hospice admissions production
Strong understanding of hospice eligibility and compliance

Preferred

Experience in the Fort Worth / Tarrant County hospital market
