About the Role
A respected hospice organization is seeking an experienced Community Liaison to drive admissions through Skilled Nursing Facility (SNF) referral relationships in the DFW/ Surrounding areas. This is a field-based role focused on building trusted partnerships, educating SNF teams, and generating ethical, compliant hospice admissions.
The ideal candidate has existing SNF relationships, understands hospice eligibility and compliance, and consistently produces admissions in a Medicare-driven environment.
Key Responsibilities
- Build and maintain strong SNF referral relationships
- Drive hospice admissions and census growth
- Educate SNF staff on hospice services and eligibility
- Coordinate referrals with intake and clinical teams
- Track referral activity and admissions performance
- Maintain strict compliance with hospice referral regulations
Qualifications
Required
- Hospice or home health liaison / business development experience
- Existing SNF referral relationships
- Proven admissions production
- Strong understanding of hospice compliance
Preferred
- Experience in the DFW / Tarrant County market
