Our client, in Boca Raton FL, is seeking a highly organized, compassionate, and proactive Patient Success Manager/Certified Health Coach to oversee daily practice operations while serving as the primary patient coach throughout our wellness programs. This individual will ensure operational excellence, maintain exceptional concierge service, and provide accountability coaching to patients using established treatment plans and protocols.

Primary Responsibilities

· Oversee daily office operations.

· Ensure smooth workflow throughout the practice.

· Develop and maintain Standard Operating Procedures (SOPs).

· Improve operational efficiency.

· Coordinate projects and deadlines.

· Identify workflow bottlenecks.

· Manage office vendors and supplies.

· Ensure regulatory compliance.

Patient Success & Health Coaching

Serve as the patient's primary point of contact after enrollment.

Provide support for:

· Dietary modification compliance.

· Supplement timing and organization.

· Lifestyle coaching.

· Sleep optimization.

· Stress management.

· Accountability coaching.

· Exercise recommendations according to established protocols.

· Habit formation.

· Encouragement and motivation.

Patient Experience

Oversee the concierge experience.

· Coordinate onboarding.

· Ensure all laboratory testing is complete.

· Verify consent forms.

· Monitor appointment schedules.

· Ensure follow-up visits occur on time.

· Resolve patient concerns.

· Improve patient satisfaction.

Lead Management & Patient Conversion

Manage every prospective patient.

Responsibilities include:

· Respond to inquiries.

· Schedule consultations.

· Track leads.

· Follow up with prospects.

· Maintain CRM.

· Improve conversion rates.

· Ensure no lead is forgotten.

Qualifications

Required

· Minimum 2 years of patient success management

· Exceptional organizational skills.

· Outstanding communication skills.

· Strong computer proficiency.

· Ability to multitask.

· Excellent emotional intelligence.

· Professional demeanor.

· Experience working in concierge or private medical practice.

Preferred

· National Board Certified Health & Wellness Coach (NBC-HWC) or equivalent.

· Functional Medicine Health Coach certification.

· Institute for Integrative Nutrition (IIN) Health Coach certification.

· Functional Medicine Coaching Academy certification.

· Experience in functional medicine.

· Concierge medicine experience..

· CRM experience.

· AI workflow experience.

Compensation

· $30 per hour depending on experience

· Paid vacation and 4 holidays per year
