A growing, patient-centered practice is seeking a Nurse Practitioner to support care within an integrative, root-cause focused model. This role works closely with physician leadership to deliver personalized care, with a strong focus on prevention, symptom support, and whole-person health.

Key Responsibilities

• Provide patient care including follow-ups, symptom management, and ongoing support
• Reinforce personalized treatment plans and educate patients on next steps
• Support patients managing side effects and complex health conditions
• Monitor progress and collaborate with the care team on adjustments
• Maintain accurate, timely clinical documentation
• Partner closely with physicians and clinical staff to ensure seamless care

Qualifications

• Licensed Nurse Practitioner (or eligible)
• Experience or interest in oncology, chronic care, or integrative medicine
• Strong communication and patient education skills
• Organized, reliable, and team-oriented
