Founding Patient Advocate – RN / NP / PA

New York City | On-Site | Early-Stage Healthcare Company

We’re recruiting for a unique opportunity for an experienced RN, Nurse Practitioner, or Physician Assistant who wants to move beyond traditional bedside care and have a direct impact on how patients and families navigate the healthcare system.

This individual will serve as the company's first Patient Advocate, working directly with seniors and their families to coordinate care, solve problems, and make sure important healthcare needs don't fall through the cracks.

This is a hands-on, high-ownership position for someone who enjoys the patient relationship, advocacy, and care-coordination side of healthcare.

What You'll Do

You'll become a central point of contact for patients and their families, helping them navigate physicians, specialists, hospitals, pharmacies, insurance companies, and other healthcare providers.

Responsibilities include:

  • Build trusted relationships with seniors and their families
  • Coordinate care across physicians and specialists
  • Manage referrals and appointment follow-up
  • Support post-hospital and discharge care
  • Follow up on testing, treatment plans, and outstanding care needs
  • Coordinate medications across providers and pharmacies
  • Help resolve prescription and pharmacy issues
  • Assist with insurance questions, authorizations, denials, and appeals
  • Communicate updates and next steps to family members
  • Identify gaps in care and proactively work to resolve them
  • Track outstanding issues through completion
  • Work closely with physicians and other healthcare professionals
  • Use technology and AI-enabled tools to organize, document, and track patient needs
  • Help develop the workflows and best practices for a growing Patient Advocate team

What We're Looking For

Required:

  • Active, unrestricted RN, NP, or PA license in good standing
  • 3+ years of patient-facing healthcare experience
  • Experience working with seniors or older adults
  • Strong care coordination, case management, or patient-navigation experience
  • Excellent communication skills with patients and families
  • Exceptional organization and follow-through
  • Comfortable working with physicians, pharmacies, hospitals, and insurance companies
  • Comfortable with technology and learning new tools
