Patient Advocate – RN / NP / PA | Senior Care

New York City | On-Site | Monday–Friday

Are you an RN, NP, or PA who enjoys working with older adults but wants to use your clinical experience outside of a traditional bedside environment?

We’re recruiting for a unique opportunity with an innovative healthcare organization focused on helping seniors and their families better manage complex healthcare needs.

This role combines patient advocacy, clinical knowledge, care coordination, technology, and relationship building. You’ll work closely with a select group of seniors and their families, becoming the person who understands the bigger picture and makes sure important healthcare needs don't get overlooked.

What You'll Do

• Develop ongoing relationships with seniors and their families
• Coordinate care between primary care providers, specialists, hospitals, pharmacies, and other healthcare resources
• Manage referrals, appointments, testing, and follow-up needs
• Support patients following hospital stays and transitions of care
• Review medication information across providers and pharmacies and help resolve discrepancies
• Assist patients and families with insurance issues, authorizations, denials, and appeals
• Keep family members informed about outstanding needs and next steps
• Identify unresolved issues and follow them through to completion
• Review technology-generated recommendations and information for accuracy
• Escalate issues requiring additional clinical decision-making to the appropriate physician
• Maintain accurate documentation of patient interactions and completed activities
• Help develop workflows, checklists, communication standards, and processes as the program grows

What We're Looking For

• Current, unrestricted RN, NP, or PA license in good standing
• 4+ years of direct patient-facing healthcare experience
• Meaningful experience working with older adults
• Experience in care coordination, patient navigation, case management, geriatrics, transitional care, home health, primary care, or a similar environment
• Strong clinical judgment and the ability to recognize when an issue needs physician involvement
• Excellent communication skills with both patients and families
• Exceptional organization and follow-through
• Comfortable communicating with healthcare providers, pharmacies, and insurance companies
• Comfortable working with technology and AI-supported healthcare tools
• Spanish-speaking is a plus

This isn't a traditional bedside position or high-volume case-management role.

You'll have the opportunity to spend your time helping seniors and families actually navigate healthcare—solving problems, coordinating resources, following up on unresolved issues, and providing continuity when multiple providers are involved.

You'll also be joining a growing healthcare organization at an early stage. Your experience will help shape how the Patient Advocate function operates as the program expands.
