About the Role
A respected hospice organization is seeking an experienced Hospital-Based Community Liaison to drive admissions through acute-care hospital referral relationships in the Fort Worth area. This field-based role partners closely with hospital case managers and discharge planners to support timely, appropriate hospice transitions in a Medicare-driven environment.
The ideal candidate has established hospital relationships, understands hospice eligibility and compliance, and consistently produces admissions.
Key Responsibilities
- Build and maintain strong hospital referral relationships (case managers, social workers, discharge planners)
- Drive hospice admissions and census growth from hospital settings
- Educate hospital teams on hospice services, eligibility, and transitions of care
- Coordinate referrals with intake and clinical teams for timely admissions
- Track referral activity and admissions performance
- Maintain strict compliance with hospice referral regulations
Qualifications
Required
- Hospice or home health liaison / business development experience
- Existing hospital referral relationships
- Proven hospice admissions production
- Strong understanding of hospice eligibility and compliance
Preferred
- Experience in the Fort Worth / Tarrant County hospital market
