We are seeking a skilled and compassionate Occupational Therapist to join a dedicated home health team serving patients across the Smithfield area. In this role, you will help patients restore function, improve mobility, manage pain, and maintain independence — all within the comfort of their own homes.
Occupational Therapist Responsibilities:
- Perform comprehensive evaluations, determine OT diagnoses, and develop individualized plans of care
- Conduct timely reassessments, including OASIS, and update care plans as appropriate
- Deliver therapeutic treatments and evaluate equipment needs
- Document clinical progress to support skilled interventions and track patient outcomes
- Coordinate with Occupational Therapy Assistants, Home Health Aides, and the interdisciplinary care team
- Modify treatment plans to maximize therapeutic benefit and achieve optimal patient outcomes
Occupational Therapist Requirements:
- Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy (required)
- Active Virginia OT License (required upon hire)
- 1 year of Occupational Therapy experience preferred
- CPR/BLS Certification (required within 30 days)
- Valid Driver’s License and ability to drive personal vehicle 25%+ of the time for patient visits
