We’re seeking an Assertive Community Treatment (ACT) Team Leader to provide hands-on clinical supervision, mentorship, and leadership to a talented team of case managers and direct service providers. This role is pivotal in supporting individuals with complex needs through integrated, person-centered care.
What You’ll Do:
- Lead and mentor a multidisciplinary team, providing both clinical and administrative supervision
- Ensure effective implementation of Health Home services such as care coordination, wellness support, and transitional care
- Support professional development through training, performance reviews, and hands-on coaching
- Oversee documentation to ensure compliance with quality standards and timelines
- Deliver direct client support when needed to foster team learning and continuity of care
- Serve as a clinical liaison with state behavioral health programs to maintain compliance and quality standards
What You Bring:
- Master’s degree in Behavioral Health, Human Services, or a related field
- Minimum 10 years of behavioral healthcare experience
- At least 4 years of leadership or supervisory experience
- Independent clinical license (LICSW, LMHC, or equivalent)
- Strong knowledge of trauma-informed care and team-based approaches
- A collaborative mindset, excellent communication skills, and a commitment to service excellence
- Valid driver’s license and access to reliable transportation
