Position Summary

ACT Program non profit organization is seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our Assertive Community Treatment (ACT) Team. This role serves as the primary psychiatric provider for adults living with serious mental illness and co-occurring substance use disorders who have historically struggled to engage with traditional outpatient services.

The PMHNP is a vital member of the ACT interdisciplinary team, providing psychiatric evaluations, medication management, crisis intervention, and ongoing psychiatric care in community settings. Services are delivered where clients live, work, and spend their time, including homes, supportive housing programs, shelters, and other community locations.

This position requires a recovery-oriented, trauma-informed clinician who is passionate about helping individuals with schizophrenia, schizoaffective disorder, bipolar disorder, major depressive disorder, and co-occurring substance use disorders achieve stability, independence, and improved quality of life.

As a full member of the ACT team, the PMHNP collaborates closely with therapists, nurses, case managers, peer support specialists, and community partners to ensure integrated, person-centered treatment that supports each client's recovery goals.
