Case Manager Responsibilities include but are not limited to:
- Travel to provide in-home and community-based services to youth and families
- Manage a caseload of foster youth and support foster homes
- Coordinate services with schools, courts, medical providers, and community partners
- Conduct assessments and assist in developing individualized service and treatment plans
- Support family reunification and placement stability efforts
- Maintain timely and accurate documentation of visits, services, and case progress
- Build and maintain strong relationships with youth, families, and stakeholders
- Participate in team meetings, court involvement, and on-call rotation as needed
Case Manager Qualifications include but are not limited to:
- Bachelor’s Degree in Social Work, Counseling, Psychology, Sociology, or related human services field
- LSW, LPC, or license-eligible preferred
- Minimum 2 years of relevant experience preferred (child welfare or working with youth strongly preferred)
- Strong communication, organizational, and problem-solving skills
- Ability to work independently and manage a flexible schedule
- Valid driver’s license, reliable transportation, and ability to travel daily
