We have an great opportunity for a CMARC Social Worker with one of the premier counties in North Carolina.
In this role, you will provide critical care coordination services to children (ages 0–5) and pregnant women who are medically fragile, developmentally at risk, or experiencing challenging social circumstances.
Your work will have a direct and lasting impact on child health outcomes and family well-being by connecting families to essential medical, developmental, and community resources.

CMARC Social Worker Qualifications include but are not limited to:
- Bachelor’s Degree in Social Work (or a related Human Services field with equivalent experience).
- NC Social Work Licensure eligibility preferred.
- Valid North Carolina driver’s license and reliable transportation.
- Experience in maternal/child health, pediatrics, or care coordination preferred.
- Strong communication, documentation, and interpersonal skills.
CMARC Social Worker Responsibilities include but are not limited to:
- Providing home-based case management services for high-risk children and pregnant women.
- Conducting assessments, developing care plans, and monitoring progress.
- Coordinating services such as developmental screenings, medical care, therapies, counseling, and social supports.
- Educating and advocating for families navigating complex systems.
- Collaborating with healthcare providers, social services, and community partners.
- Maintaining documentation that meets Medicaid and state care coordination standards.
