We are a pediatric practice seeking a Patient Care Coordinator or Care Navigator to join our growing team!!
Patient Care Coordinator Benefits:

Patient Care Coordinator Responsibilities:
- Provide direct individualized case management services to patients
- Assist families with various applications as well as navigating many other systems and agencies
- Identify and develop innovative and streamlined ways to help patients overcome barriers
- Partner with established community organizations and develop new partnerships
- Collaborate with providers to deliver holistic and supportive primary care
- Participate in various special events throughout the year
- Complete detailed case notes
- Brainstorm solutions with the care navigation team
Patient Care Coordinator Requirements:
- Bachelor's Degree in a related field (education, public health, social work, etc.)
- Proficiency in Spanish required
- 2+ years working with children
- Excellent teamwork skills
