Your Role

As a Home Health Social Worker, you will:

• Assess clients’ needs and develop individualized care plans.
• Connect clients to resources like financial aid, counseling, and healthcare support.
• Provide emotional support and guidance to patients and their families.
• Collaborate with a multidisciplinary team to ensure the best outcomes for clients.

What You Bring

• Education: Master’s in Social Work (MSW) preferred, Bachelor’s with experience considered.
• Licensure: Active Colorado LCSW required.
• Experience: 1+ year in a healthcare or home health setting (preferred but not required).
• Skills: Strong communication, cultural competency, and a heart for service.
