We are seeking an experienced Case Manager (QIDP) to join our passionate team. We believe in respect and individualized care as we support our residents in their homes.

Case Manager (QIDP) Responsibilities:

• Oversees resident's program needs, goals, and community interaction
• Ensures quality care for residents
• Creates programs to nurture the growth and independence of the residents

Case Manager (QIDP) Requirements:

• 1+ year of experience working with persons with developmental disabilities
• Bachelor's degree required
  • preferably in human services, psychology, sociology, special education, rehabilitation, etc.
• Driver's license required
• Excellent communication and collaboration skills
