LPN Requirements:
- Active LPN license in Colorado
- Proof of COVID-19 vaccination and booster
- 2+ years of home health or LPN experience
- Reliable vehicle for rural travel
LPN Responsibilities:
- Deliver skilled care to patients in assigned regions
- Collaborate with patients, families, and care staff on care plans
- Communicate patient updates with the interdisciplinary team
- Maintain daily charting and coordinate with physicians
- Attend required case meetings
