Medical Social Worker Responsibilities:

·       Provides assessment and follow-up social work services

·       Contributes to collaborative IDT process

·       Assists participants and families with needs for community supports

Medical Social Worker Qualifications:

·       Master's degree in Social Work

·       Current driver's license

·       2 years experience in senior case management, hospital or long-term care

Medical Social Worker Compensation and Benefits:

·       Competitive salary $60-$68K

·       Comprehensive benefits package

·       M-F schedule 8-4:30
