RN Case Manager Responsibilities

  • Provide ongoing assessment, professional nursing care, and coordination – In Field in patients homes
  • Delegate care tasks as appropriate
  • Work with patients, family members, and IDT to develop and implement appropriate care plans

RN Case Manager Requirements

  • Licensed RN in Good Standing
  • COVID-19 Vaxed and Boosted
  • 2+ Years of RN experience
  • Valid Driver's License and reliable transportation

RN Case Manager Compensation

  • Competitive pay 61- 91K doe
