Our expanding presence in Northern Virginia has created an exciting opportunity for a passionate and dedicated Elderly Care Manager to join our team. We are committed to helping seniors age in place by providing them with comprehensive support and services tailored to their unique needs. Elderly Care Manager Responsibilities:
- Meet clients to coordinate and assess their care needs.
- Liaise with patient facilities, physicians, and family members to ensure their well-being.
- Schedule and attend appointments, while also managing care plans for clients.
- Collaborate closely with team members for additional support and maintain excellent client service.
Elderly Care Manager Qualifications:
- Has a deep passion for working with disabled adults and senior citizens
- Demonstrated knowledge of aging, disease processes, and safety concerns.
- Familiar with various forms of dementia and the challenges related to the disease.
- Master's Degree in a health-related field or LSW.
- Must be vaccinated.
- A resident of Fairfax, VA.
