Are you a dedicated and experienced Registered Nurse seeking a meaningful career in home health care? We are an established and highly respected interdisciplinary team committed to delivering quality care in the beautiful mountainous regions outside of Denver.
Home Health RN Responsibilities:
- Provide skilled care to patients in the assigned region, either full- or part-time.
- Assess and admit new patients weekly and attend meetings as needed
- Collaborate with patients, families, and care staff to develop and implement care plans
- Maintain clear communication with the interdisciplinary team regarding patient status and changes
Home Health RN Requirements:
- Current, active RN license in good standing with the State of Colorado
- COVID-19 vaccination (proof required)
- 2+ years of Home Health or RN experience
- Reliable vehicle and valid driver's license for travel in rural landscapes
