Seeking a Senior Care Manager to join our growing presence in Northern VA, We are looking for someone to join our team of passionate coordinators helping seniors age in place with everything they could possibly need. Are you a Go-Getter? Do you love working with Seniors!! APPLY TODAY!!
Senior Care Manager Responsibilities:
- You will meet clients where they are to coordinate their care needs
- You will advocate on the client's behalf with facilities, physicians and family members
- You will attend appointments, schedule meetings and manage care plans for clients
- You will coordinate and Collaborate with team members to find support and ensure excellence in client service
Senior Care Manager Qualifications:
- You will have Experience and passion for working with senior citizens and/or disabled adults
- You will have Knowledge of aging, disease processes and safety concerns in terms of mobility and transportation
- You will have Knowledge and experience with different forms of dementia and the challenges associated
- You will have a Master's Degree in a health-related field or a Licensed Social Worker
- You must have a current vaccination record and reliable transportation
- You must live in Fairfax County, VA
