Job Summary
The MDS Coordinator must be a registered nurse certified in MDS coding; he/she is responsible for coordinating the comprehensive assessment of residents. You must have knowledge of CMS regulations. You must understand the GA Case Mix and have the ability to work with the Interdisciplinary team.
MDS Coordinator Duties
- Ensures compliance with policy and procedures for meeting the federal guidelines of Resident Assessment Instrument (RAI), which assesses a resident’s behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized care plan that considers the resident holistically
- Assesses and reassesses residents as required by federal and/or state regulations and as needed to develop a nursing care plan for each resident
- Complete MDS to initiate the process that plans, organizes, and evaluates nursing care according to established policies and procedures
- Publishes a monthly calendar or schedule assessments as required by federal and/or state regulations for team members
- Confers with clinic and therapy staff, to develop, implement and evaluate care plans
- Schedules and participate in care conferences with residents and families
- Participate in resident care conferences
MDS Coordinator QUALIFICATIONS:
- Registered Nurse with a current State of Georgia license
- Minimum of two-year MDS experience, MDS certification preferred
- Minimum of two years long term care experience
- Minimum of two-year Case Mix experience
