Our established and reputable home health interdisciplinary team is seeking an experienced LPN to round out our care teams servicing the mountain/rural regions outside of Denver. We are passionate about bringing care to all of those who need it and that includes the beautiful mountainous regions and hard-to-reach folks. We get there! If you are an experienced and licensed practical nurse who loves a challenge and appreciates a great team – APPLY TODAY!
LPN Responsibilities
- Work full- or part-time bringing skilled care to patients within the assigned region
- Work with patients, family members and other care staff to develop and implement appropriate care plans
- Communicate with interdisciplinary team regarding patient status, observations and changes in condition
- Chart daily and communicate as needed with primary care physician
- Attend case meetings as required
LPN Requirements
- Licensed LPN – Current and in Good Standing with State of Colorado
- COVID-19 Vaxed and Boosted (Must show proof)
- 2+ Years of Home Health or LPN experience
- Proven ability to adhere to visit schedule and demonstrate a commitment to quality patient care
- Valid Driver's License, Insurance and reliable vehicle able to traverse challenging and rural landscapes
